An exit meeting was completed by the licensing inspector with the provider to review the inspection findings.
The licensing inspector completed an unannounced renewal inspection on Monday, August 21, 2017 from approximately 11:15 am to 1:36 pm to determine the provider's compliance with licensing standards. Upon the licensing inspector's arrival, the children were observed at the family day home's outdoor playground engaged in play and interacting with peers and their caregivers. The licensing inspector received assistance from the provider throughout the inspection. During this inspection, there were 9 children totaling 20 points being cared for by three caregivers including the provider. The children in care were observed by the licensing inspector participating in various activities to include playing with age appropriate toys, playing the Uno card game, interacting with their peers, and setting the table for lunch. The licensing inspector completed formal observations as the children were preparing for lunch by completing handwashing, saying their grace, and sitting at the table and/or high chairs eating their lunch. The children's lunch today consisted of spaghetti, salad, green beans, carrots, and milk. During the inspection, the children interacted with their peers, caregivers, and the licensing inspector during their lunch time. The licensing inspector also interacted with the caregiver and the children when appropriate. After the lunch time, the children completed bathroom breaks, watched Octonauts, played with toys, and transitioned into napping. The licensing inspector observed the caregivers completing diapering during this inspection as well. The diapering station was inspected and deemed in compliance with licensing standards. The caregivers were observed providing age appropriate supervision and addressing children's overall needs. During this inspection, the bathroom, kitchen, and designated childcare areas were inspected. The designated areas for childcare were deemed appropriate and equipped with age appropriate toys and materials to meet the ages and stages of development of the children. The licensing inspector completed observations of two parent pick ups during this inspection as well. The inspector reviewed the required postings to include license, prior inspection summary, emergency contact information and breakfast and lunch menus. The first aid kit and emergency supplies, fire and shelter-in-place drills, thermometers, and emergency preparedness plan were inspected and determined to be in compliance with the licensing standards. The emergency preparedness plan updated on August 1, 2017. There were five children's, the provider's, two substitute provider's/assistant's, and household member's records reviewed for compliance of standards. There are currently no medications being administered by providers. The violations from the previous inspections were checked for corrections.

Standard #: 22VAC40-111-100-A Description: Based on review of children's records, the provider did not ensure documentation of a physical examination was obtained within 30 days after the first day of attendance.
Evidence: In the record of Child #3, there was no documentation of a physical examination found. Child#3's start date was recorded as 04/06/16. The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Plan of Correction: Provider will contact parent and have document completed by the physician.
