Comments:
An unannounced renewal inspection was completed on 8/22/2017. The inspector arrived at 8:45A.M. and departed at 11:00 A.M. Seven children were in care at 19 points with the licensee and one assistant caregiver. Two caregiver records were reviewed. Five children's records were reviewed. A physical plant walk-through was done. Policies and procedures were reviewed. Discussion: subsidy requirements, standards, new sworn disclosure statement.

Violations:
Standard #: 22VAC40-111-760-A
Description: Based on observation of the first aid kit used in the vehicle to transport children, the center failed to ensure that it was complete. Evidence: 1. The first aid kit that is used in the vehicle for transporting children did not contain all of the required items. The following items were missing: two triangular bandages and a pair of scissors.

Plan of Correction: The licensee will make the bandages and will add the missing items.

Standard #: 22VAC40-111-60-A
Description: Based on review of five children's records, the licensee failed to ensure that all records were complete. Evidence: 1. The inspector found that children 2, 3 and 4 were missing the start dates 2 Child # 5 was missing one of the emergency contacts addresses.

Plan of Correction: The licensee completed the start dates during the inspection and will obtain the missing address.

Standard #: 22VAC40-111-520-B
Description: Based on observation, the licensee failed to ensure that all cribs were not used to for a play space for infants. Evidence: 1. One one 11 month of child sitting in his crib from 8:58-9:10 A.M and from 9:30 to 10:00 A.M.

Plan of Correction: The licensee will use a pack and play to ensure that child's safety while older children are playing.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the provider did not ensure that a caregiver does not exceed 16 points. Evidence: 1. The assistant was alone with 19 points from 8:35 A.M. to 9:19 A.M. The licensee was transporting children to school. Two children at four points each, one child at three points, four children at two points each.

Plan of Correction: The licensee will look at points prior to leaving and will make sure that points are maintained.

Standard #: 22VAC40-191-40-D-1-c
Description: Based review of staff records, the licensee failed to ensure that all backgrounds were repeated every three years. Evidence: 1. Staff # 2 most recent CRC is 3/12/2017, there is documentation that the CRC was returned 4/25/2017. As of the day of the inspection the CRC has not been returned.

Plan of Correction: The licensee will call to follow up the status of the background check.
