An announced initial study was conducted from 10:15 - 11:45 am. There were no children in care during the inspection. The family day home was observed for age/stage appropriate supplies, activities, and equipment. The provider and two household member records were reviewed and in compliance. One pet record was reviewed and in compliance. The first aid kit, flashlight, and weather band radio was reviewed and in compliance. The provider is current in CPR and First Aid. Description: Based on observation of the outdoor play area it was determined that not all stationary play equipment was placed at least six feet away from the perimeter. Evidence: The swing set in the back yard was located between 3 - 4 feet from the wood timbers and concrete sidewalk.

Plan of Correction: The playground equipment will be moved back to create a six foot perimeter.
