An unannounced renewal study was conducted today. There is one child present with 2 caregivers (2 points). 7 adult and one child's file were reviewed. See violation notice for missing information. The child was observed playing with a variety of toys during the inspection. Standard #: 22VAC40-111-60-A
Description: Based upon a review of files, one child's file was not updated annually. Evidence: Child A's enrollment form is dated 8/18/16, expired.

Plan of Correction: I will ask the parent to review the form and sign it again.

Standard #: 22VAC40-111-90-C
Description: Based upon a review of files, one child's file is incomplete. Evidence: Child A is missing an updated immunization record, every 6 months until the age of 2. Immunizations on file are dates 8/15.

Plan of Correction: I think the mother has it, I will ask her for it.

Standard #: 22VAC40-111-180-A
Description: Based upon a review of files, adult files are incomplete. Evidence: 6 of 7 files reviewed were missing documentation of a current TB test. (Adult 1, 2, 3, 4, 5, 6)

Plan of Correction: I will have the TB test redone or a new screening.

Standard #: 22VAC40-111-240-A
Description: Based upon direct observation, the day care area is not maintained in a safe/clean manner. Evidence: There is apparent evidence of mold growing on the wood paneled wall to the right of the day care entrance, along the baseboard.

Plan of Correction: I will check into having the area cleaned or repaired.
