Technical Assistance:
A supplemental page was provided. It is my understanding that the provider is not using the playground with the equipment until more mulch can be added.

Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, August 28, 2017. The on-site inspection was conducted from 11:05 a.m. to 3:00 p.m. On this date, there were six children in the care of the provider and assistant (15 points). There were a total of 13 children enrolled in the program. There were five children's records reviewed and the records of three caregivers and one adult household member were reviewed for updates. The provider, the assistant and three children were interviewed. There were no medications being administered at the time of the inspection, which is the policy of the provider. The violation from the previous inspection was reviewed for compliance. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. Lunch was observed to meet USDA guidelines. Some items reviewed and observed included emergency evacuation drills, posted information, first aid kit and emergency supplies, choice time, diapering procedures, bathroom and hand washing procedures, infant feeding, lunch, and nap time. Violations:
Standard #: 22VAC40-111-210-A
Description: Based on record review and verification from the provider, two caregivers failed to get the required 16 hours of annual training. Evidence; 1. Caregiver #2 obtained 8.5 hours of training from June 2016 to June 2017. 2. Caregiver #3 obtained 14.5 hours of training from May 2016 to May 2017. 3. The provider verified that the two assistants had not obtained their 16 hours of annual training.

Plan of Correction: The two caregivers will obtain the remaining hours of training needed and then will obtain 16 hours of training for the 2017 - 2018 year.
