Reminder: household criminal history record checks are due to be repeated 3/2018 unless national fingerprinting procedures have been implemented before then. Look for e-mail with updated information this Fall.
An unannounced monitoring inspection was conducted with provider. There were two children present totalling 6 points. The children were arriving to care and playing with age appropriate toys. The basement has many age appropriate toys and activities and equipment. Three children's files and one household file were reviewed - please see Supplemental Summary for missing items. Provider does not administer medication - Medication Administration Training is required to administer medication. Provider renewed first aid and CPR certification 2/11/2017. Standard #: 22VAC40-111-100-A Description: Based on review of three children's files, the provider failed to have documentation of a physical within 30 days of enrollment. Evidence: Child B (date of enrollment 12/12/2016) and Child C (date of enrollment 2/2/2017) did not have documentation of a physical within 30 days of enrollment. Plan of Correction: Provider will ask the parents for the physicals.
Standard #: 22VAC40-111-90-A Description: Based on review of three children's files, the provider failed to have documentation that the children are adequately immunized before attending the family day home. Evidence: Child C did not have documentation of immunization record on file. Plan of Correction: Provider will ask parent for the immunization record.
Standard #: 22VAC40-111-180-A Description: Based on review of one household file, the provider failed to have documentation of a repeat tuberculosis test or screening within two years of the previous screening. Evidence: the provider could not locate documentation of a current TB test or screening. Plan of Correction: Provider will obtain documentation of a current TB test.
