Comments:
An unannounced monitoring inspection was conducted today from approximately 10:00am - 11:40am. There were 4 children (8 points) directly supervised by 1 caregivers. A sample of 4 children?s and 1 staff records were reviewed. The children were observed walking from local playground, eating lunch, and playing together. Lunch served consisted of pasta and vegetables. No new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-920
Description: Based on observation the menu for the current one-week period was not maintained. Evidence: The menu for the weeks of 08/07/2017 and 08/14/2017 were not available for review.

Plan of Correction: The food menu will be posted monthly

Standard #: 22VAC40-111-120-B
Description: Based of record review a substitute's records did not contain all required documents. Evidence: Missing information includes: two required references

Plan of Correction: References will be on hand upon request

Standard #: 22VAC40-111-60-B
Description: Based on record review children's records were missing some required information. Evidence: 4 out of 4 records reviewed were missing some required information. 1) Child A - missing medical insurance policy number, address for 1 of 2 emergency contacts, missing physician signature on immunization and physical exam. 2)Child B - missing first day of attendance, 3) Child B and C - missing annual update and review of record 4) Child D - missing last immunization.

Plan of Correction: Correction will be done as soon as posssible

Description: Based on observation current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department was not maintained. Evidence: Substitute's first aid training expired February 2017 and exceeded the three year deadline for recertification.

Plan of Correction: Recertification will be done before 9/16/17

Standard #: 22VAC40-111-150-D-1
Description: Based on observation current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department was not maintained. Evidence: Substitute's current CPR certification expired February 2017 and exceeded the two year deadline for recertification.

Plan of Correction: CPR certification will be obtained. Substitute will schedule training.

Standard #: 22VAC40-111-280
Description: Based on observation potentially poisonous substances, materials and supplies such as cleaning agents were not inaccessible to children. Evidence: 1) Closet near the cribs located in the basement (primary child area) contained cleaning and disinfectant agents that were accessible to the children in care. 2) The cabinet under the sink on the entry level where children play and eat was contained cleaning agents and disinfectants. There was a lock on the cabinet, however it was broken, making the cleaning and disinfecting agents accessible to children in care.

Plan of Correction: Cleaning and disinfectant agents will be removed. Lock will be replaced and secured.
