Comments:
An unannounced monitoring inspection was conducted from 11:00 - 1:45 p.m. There was one child in care with the provider. The provider had two points. There are three children enrolled and three children's records were reviewed. The record for the provider was reviewed. The child was observed talking with the provider, playing with toys, skipping and dancing, playing pretend, identifying different fish and discussing how many fish of each kind there were, counting, washing her hands before and after lunch, during lunch and resting. Areas of non-compliance are identified on the violation notice. Should you have any questions please telephone your licensing inspector at (540) 430-9257.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on documentation the facility failed to have documentation of a monthly evacuation drill in August 2017. Evidence: 1. On 09/12/2017 the licensing inspector reviewed the documentation of monthly evacuation drills. There was no monthly evacuation drill documented for August 2017. 2. On 09/12/2017 the provider confirmed she missed conducting a monthly evacuation drill in August.

Plan of Correction: I will put this on the calendar to make sure I do not miss any further monthly evacuation drills.

Standard #: 22VAC40-111-60-B
Description: Based on record review the facility failed to have written documentation of a written parent acknowledgment of the annual review of emergency contact information. Evidence: 1. On 09/12/2017 the record for child #3 was reviewed. There was no written documentation of the parent's acknowledgment of the annual review of emergency contact information. 2. On 09/12/2017 the provider confirmed there was no written documentation of the parent's acknowledgement of the annual review of emergency contact information.

Plan of Correction: This child had a lapse in care due to the parent not attending school which resulted in my not having covered the emergency contact information. I have laid this out to go over with the parent this evening at pick-up.
