A Subsidy Health and Safety inspection was conducted on 09/19/2017 from 11:30am to 1:45pm to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDHOM-000-005-B Description: Based on review, the provider did not maintain a hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The list of children that were present each day did not have arrival or departure times for the children. Plan of Correction: I will document the arrival and departure times on my roster.
Standard #: VENDHOM-000-029-B-7 Description: Based on review, the caregiver did not have documentation of a yearly review and update of the Emergency plan. Evidence: 1. The Emergency plan had documentation that it was reviewed on 12/20/2015. 2. The caregiver stated that the plan had been reviewed this year, but could not find documentation of the review. Plan of Correction: I will update the emergency plan.
