Today we discussed choking hazards, tuberculosis screening, reference checks and fireplace inspections.
An unannounced renewal inspection was completed today from 11:00am and ended at 12:15pm. At the end of the inspection there were 8 children in care with 2 staff. Children were observed eating lunch, participating in age appropriate activities, hand washing, lunch and nap. The sample size was 5 children's records and 2 staff records where reviewed. The violations cited were in the area of choking hazards, tuberculosis screening, staff reference checks, annual fireplace inspection and appear on the violation notices.
Standard #: 22VAC40-111-120-B Description: Violation: Based upon record review and interview the licensee did not provide documentation of two or more references as to character and reputation for hired child care staff. Evidence: 1 ? During review of staff records two references were not provided for staff #2. a. Staff #2 has a documented date of hire as 8/14/17. 2 ? Staff#1 confirmed that there were not any references on file for staff#2. Plan of Correction: The provider will confirm that staff provide two references by the date of September 15, 2017.
Standard #: 22VAC40-111-170-B Description: Violation: Based on record review and interview the provider failed to ensure that the tuberculosis screening form is completed within the last 30 days. Evidence: 1 ? The record for staff 1 does not contain documentation of a tuberculosis screening within the last 30 days. a. Staff# 2 has a documented date of hire as 8/14/17. b. The tuberculosis screening in the record is dated 6/22 /16. 2 ? The provider confirmed that the tuberculosis screening was not completed within the last 30 days. Plan of Correction: The provider will confirm that staff will have a current tuberculosis screening form. The provider advised tuberculosis screening form will be updated by September 15, 2017.
Standard #: 22VAC40-111-250-B Description: Violation: Based on observation and interview the provider did not ensure that children were protected from materials that could be swallowed or present a choking hazard. Evidence: 1 - During the observation it was found that several small foam star shaped objects were accessible to children less than the age of 3 years old. a. The star shaped choking hazard objects were located on the floor of the children?s play room. b. When the items were placed in a ?No Choke Tube Tester? each fit fully inside the tester and failed the test. Therefore, the items present a choking hazard 2 ? The provider confirmed that the items failed the Choke Tube Test and were a choking hazard for the children in care. Plan of Correction: Provider will use a choke tube tester to determine if additional items should be removed from the accessible toys.
Standard #: 22VAC40-111-320-G Description: Based upon documentation review and interview the provider failed to ensure that wood burning fireplaces and associated chimneys are inspected annually. Evidence: During the inspection of the family day home the provider could not make available documentation showing the annual fireplace inspection. Plan of Correction: Provider will schedule an appointment for a fireplace inspection and received documentation of inspection by the date of September 29, 2017.
