Thank you for your assistance during this unannounced monitoring inspection. This on-site inspection was conducted on September 26, 2017 from 11:30 a.m. through 2:00 p.m. Upon arrival, there were four (4) children in the care of the provider with points totaling ten (10). Four (4) children's and the records for the provider and one household member was reviewed and updated. No medication is being administered at this time. Areas observed were nap, free choice activity, hand washing/diapering/ bathroom procedures and story time. The interactions were observed as positive and the children seem comfortable with the daily routine and schedule. If I may be of assistance please contact me at (540) 430-9256.

Description: Based on record review the provider and one household member had not obtained an updated tuberculosis screening. Evidence: 1. The records were reviewed for the provider and one household member. 2. The most recent documentation for an updated tuberculosis screening for the provider was dated 09/08/2015. 3. The most recent documentation for an updated tuberculosis screening for the provider was dated 08/28/2015. 4. The provider confirmed updated tuberculosis screenings had not been obtained.

Plan of Correction: I will have an updated tuberculosis screening completed for myself and my husband.
