Description: Based on household member files reviewed, one household member did not have a current Tuberculosis Screening on file. Evidence: At the time of inspection, Household Member A most recent TB screening is dated 3/13.

Description: Based on observation, potentially poisonous substances were not stored away from food. Evidence: 1) During inspection there was a cleaning spray bottle found on the same shelf as pretzels in the cabinet under the sink in the. kitchen area 2) Hazardous substances were found out side in the backyard on a cart and on a brick wall near play equipment and accessible to children in care.

Plan of Correction: Will move hazardous substances into a locked area. Completed during inspection.

Description: Based on observation, Provider exceeded 16 points. Evidence: During time of inspection there were 7 children in care with a total of 18 points with the provider as the only caregiver.

Plan of Correction: Provider will train daughter to become an assistant.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was found no longer absorbent. Evidence: The diapering pad was found to be ripped and no longer absorbent.

Plan of Correction: Provider will replace the diapering pad.
