- Technical Assistance:
Discussed cleaning agents and outlet covers that are not accessible to children. It was also discussed that children cannot sleep with pacifiers that are attached to a stuffed animal.
Today an unannounced Monitoring inspection was conducted. This visit took place in the afternoon between 12:00pm and 2:20pm. There were 8 children present with the provider and 1 assistant for a total of 20 points within the supervision guidelines. The children were observed playing, dancing, having bottles and going down for naps. Positive interactions with the children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Thank you for your cooperation. Standard #: 22VAC40-111-120-A Description: Based on record review, staff files did not have all of the required information. Evidence: Staff file #2 did not have the employee's first date of employment, emergency contact information, or two references. Plan of Correction: I will have her fill in the missing information. Provider states that references were completed but not documented.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all of the required information. Evidence: Child #1 is missing authorized pick-ups. Child #2 is missing their first date of attendance. Child #3 is missing their medical insurance policy number. Child #4 is missing their first date of attendance and their medical insurance policy number. Plan of Correction: When the parents pick up I will make sure they will complete the forms.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review, there was no documentation of current Cardiopulmonary Resuscitation (CPR) certification for the provider. Evidence: The providers CPR certification expired on 1/31/2017. Plan of Correction: Provider states that her CPR training is complete, she will continue to look for the certificates.
Standard #: 22VAC40-111-140-B-4 Description: Based on record review, there was no documentation of current First Aid certification for the provider. Evidence: The providers First Aid certification expired on 1/31/2017. Plan of Correction: Provider states that her First Aid training is complete, she will continue to look for the certificates.
Standard #: 22VAC40-111-180-A Description: Based on record review, household members did not have a current Tuberculosis (TB) Screening. Evidence: The provider's TB Screening expired on 11/11/2016. Household member #1 TB Screening expired on 11/17/2016. Plan of Correction: We will get it completed over the weekend.
Standard #: 22VAC40-111-200-A Description: Based on record review, there was no documentation of orientation for an assistant. Evidence: There is no orientation documented in their file (Date of hire: April 2017). Plan of Correction: The provider states that orientation was completed, we will get it documented.
Standard #: 22VAC40-111-480-D Description: Based on observation, equipment was not being used in accordance to the manufacturer's instruction. Evidence: A mattress for one of the cribs in the nap room was not on the platform, but flat on the floor, within the confines of the crib frame. Plan of Correction: This was corrected during the inspection. The provider states that she will be purchasing a cot for this child.
