Technical Assistance:
Provider will need to conduct one lockdown drill per year.

A Subsidy Health and Safety inspection was conducted on September 26, 2017 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please send questions regarding this new requirement to .

Description: Based on record review and interview, it was determined the vendor's daily attendance record does not include the arrival and departure of each child in care as it occurs. Evidence: 1. The Inspector reviewed the daily attendance record and it did not include arrivals and departure times. 2. Provider stated she did not realize she needed to include the times.

Plan of Correction: I will record in and out times.

Standard #: VENDHOM-000-012-B
Description: Based on interviews, it was determined the vendor provider has not, within 90 days of service, completed Virginia Pre-Service Training for Child Care Staff. Evidence: 1. FDH has been enrolled in the subsidy program for over 90 days and the caregivers have not taken the 10 hour online Pre-Service course.

Plan of Correction: We will enroll and take it this week.

Description: Based on record review, it was determined the vendor provider's Emergency Preparedness plan is missing lockdown procedure including where to go into home.

Plan of Correction: We will include the information in the Plan today.
