An unannounced monitoring inspection was completed on 09/29/2017. There were 9 children, ages 5 months-4 years, totaling 26 points in care today. The provider and an assistant were on-site providing direct supervision during today's inspection. The inspector reviewed 5 children's records, a provider, an assistant and 2 household member's records during the inspection. The provider does not administer medication. The children were observed in the following activities: outside play, music and movement, story time, lunch, and nap time. Infants were observed napping and being fed on demand. The inspector discussed the annual trainings provided by Smart Beginnings, completing a monthly fire drill and the new background check requirements with the provider. An exit meeting took place at the conclusion of today's inspection to discuss areas of non-compliance. The inspector arrived at 8:45 a.m. and departed at 1:00 p.m. Thank you for your time today. Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider failed to obtain documentation of a physical examination within 30 days of the first day of attendance. 1. The record for Child #3 did not contain a physical examination; the start date for Child #3 was 07/31/2017.

Plan of Correction: The provider will get the physical from the parent.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider failed to obtain documentation that the child has been adequately immunized according to the requirements of the Code of Virginia. Evidence: The record for Child #3 did not contain documentation of the child's immunization records; the start date for Child #3 is 07/31/2017 and was in care during today's inspection.

Plan of Correction: The provider will get the immunization record from the parent.
