An unannounced monitoring inspection was completed on 10/3/2017 beginning at 2:45 P.M. and ending at 4:45 P.M. The licensee was present with the licensee's one infant one child in care children. The points were at five and in compliance. The LI reviewed two children's and one staff and a household member's record during the inspection. Observation of the children during: free play, TV time, and snack. Discussion about upcoming changes to the licensee's household, and future plans for day care schedule.

Description: Based on review of two children's records, the license failed to ensure that all records were complete. Evidence: 1. The inspector found that child # 2 with the start date of 10/2/2017 did not have the Policy for administration of medication and the provision of home's emergency preparedness and response plan were not signed by parent or guardian. All required paperwork is to be completed by the time the child's starts the program.

Plan of Correction: The licensee will have the guardian to sign all required paperwork as soon as the child's returns. All required paper work will be complete by the time child's starts the program.

Standard #: 22VAC40-111-240-A
Description: Based on observation the licensee failed to ensure that all areas and furnishings of the family day home were maintained in a clean, safe and operable condition. Evidence: 1. The inspector found that the gutter outside the front door in the outside play area was cracked and had sharp edges that could snag the skin of the children in care.

Plan of Correction: The licensee will have a family member to fix the gutter.
