A Subsidy Health and Safety inspection was conducted on 10/06/2017 to review supplemental health and safety requirements. An unannounced renewal inspection was conducted today from approximately 12:35pm-1:50pm. There were 5 children (15 points) directly supervised by 2 caregivers. A sample of 3 children?s records, 2 staff records and 2 household member records were reviewed. The children were observed eating lunch, napping, and playing with one another. The outside playing area was well maintained. Zero new staff has been hired since the last inspection. The exit interview was conducted with the provider. The provider has forfeited her subsidy contract. If you have any questions, I can be reached via e-mail at Kimberly.Sawyer@dss.virginia.gov or by phone at 571-835-0599. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.
