22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted on 10/11/2017 from 8:30am-9:30am. At the time of inspectors' arrival there were 3 infant to preschool aged children in care totaling 13 points with the provider. Children were observed participating in indoor play, snack, transitioning between activities and hand washing. A sample of 4 children's records and 1 provider record was reviewed. Medications are not being administered at this time. There have been no injury reports since the last inspection. Areas of noncompliance are detailed in the violation notice and were discussed with the provider at the conclusion of this inspection.
Standard #: 22VAC40-111-100-A Description: Based on record review it was determined that the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The record for child #4(enrolled 8/2017) did not contain documentation of a physical examination. Plan of Correction: The provider will request from the parent at pick-up.
Standard #: 22VAC40-111-60-B Description: Based on record review it was determined that the provider did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child #4(enrolled 8/2017) did not contain the insurance policy number for the child, the addresses of the 2 required emergency contacts. 2. The record for child #5 and #6(enrolled 1/23/17) did not contain the complete address for 1 of the 2 required emergency contacts. Plan of Correction: Provider will request this information from the parents at time of pick up.
Standard #: 22VAC40-111-90-C Description: Based on record review it was determined that the provider did not obtain documentation of additional immunizations once every six months for children under the age of two. Evidence: The record for child #1(DOB 3/16/15) did not contain up to date documentation of immunizations. The documentation available was dated 9/1/16. Plan of Correction: The provider will request from the parent at pick up.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review it was determined that the provider did not have current certification in cardiopulmonary resuscitation (CPR). Evidence: The provider's CPR certification expired on 8/2/17. Plan of Correction: The provider will enroll in and complete a CPR class.
Standard #: 22VAC40-111-240-A Description: Based on observation it was determined the provider did not ensure that all areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Evidence: The small white and small black bookcases were not anchored and rocked when pulled on presenting potential tip/injury hazards. Plan of Correction: Provider will anchor both bookcases.
