22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced renewal inspection was conducted today from 9:45am-11:00am. There were 7 children (16 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed fingerpainting, completing puzzles, dancing and singing. The caregivers speak to the children in both Spanish and English. There was an sufficient supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did not maintain complete and up to date records for each enrolled child. Evidence: 1) Child A is missing documentation of an annual review of the emergency contacts. 2) Child B is missing documentation of annual review of the emergency contacts. 3) Child D is missing written authorization for emergency medical care should an emergency occur. Plan of Correction: I will tell parents to review it and sign.
Standard #: 22VAC40-111-70-A Description: Based on record review, the provider did not have documentation of parents being provided all required information prior to the child's first day of attendance. Evidence: Child C (date of enrollment July 2017) is missing signed acknowledgement of the medication policy, emergency preparedness plan, liability insurance, hours of operation, daily schedule, discipline policies, schedule of payment and fees, and check in and out procedures. Plan of Correction: I will have parents sign the policies today.
Standard #: 22VAC40-111-240-A Description: Based on observation and record review, the provider did not maintain all furnishings in a safe condition. Evidence: In the second sleeping room, Crib 3 has a manufacture date of June 19, 2010 which does not meet the CPSC crib standards. Plan of Correction: I will get a buy a new crib and remove the old one.
