Comments:
An announced initial family day home inspection was conducted on Thursday, October 12, 2017 from approximately 11:00 AM until 1:15 PM. The provider has requested a license to provide care for five children, from birth to 12 years of age. A complete walk through inspection of the home and outdoor areas was conducted with the provider. At the current time, there are four children in care. The background checks for the provider were reviewed during the inspection.

The majority of care and supervision of children will take place in the upstairs playroom and in a room located on the first floor. A bathroom is located on both floors of the home and has the required items. A sufficient number of toys and equipment was observed located in the home for the requested number of children. Food will be provided for the children and a weekly menu was reviewed during the inspection. The provider stated medication will not be administered at this time. Transportation will be provided and documentation and requirements for transportation was reviewed during the inspection. During the inspection, the licensing inspector reviewed emergency plans, attendance logs, parent information, required provider training, emergency supplies and required postings.

The provider has submitted all of the required application materials at this time. Though, some standards of care could not be determined during the initial study, the provider is in substantial compliance with the standards that could be determined. A recommendation for a six-month conditional license will be made at this time. During the conditional period, the provider will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure. Please contact Lynn Powers at (804) 662-9790 if you have any questions regarding this inspection.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation and interview, the first aid kit did not contain all of the required items. Evidence: During the inspection, the licensing inspector observed the first aid kit did not contain tweezers. The provider stated she did not have tweezers present in the house during the inspection.

Plan of Correction: I will buy a pair of tweezers by the end of today.
