An unannounced monitoring inspection was completed on 10/12/17. There were four children in care at arrival of the inspector and two more children arrived at approximately 9:30 am. The point total for the six children was 16. The provider was the caregiver providing the care and supervision of the children. Six children's records (two newly enrolled children) were reviewed, the provider's record was reviewed, and a newly hired assistant caregiver's record was reviewed. No medications at the family day home. The inspector arrived at the provider's home at 9:00 am and departed at 1:00 pm. The children were observed during free time, during a craft activity, during a music and movement activity, during learning time (nursery rhymes), during lunch, and during nap. There was discussion with the provider about the following: wording/details of the lock down plan (reunification), ensuring timely additions to attendance when children arrive at the family day home, reviewing and updating the emergency preparedness plan by March 2018, household member's TB test update February 2018, new Sworn Statement form, upcoming provider training on Saturday in November, completing and documenting a second shelter-in-place drill, updating children's records in January 2018 to include current school and grade level, Standard 520-B (use of a crib for a play space), details of use of an assistant and being able to leave the assistant alone.
Standard #: 22VAC40-111-250-C Description: Based on observation, the provider failed to ensure that items tied across the top or corner of a crib or playpen or toys hung from the sides with strings or cords were removed when the child begins to push up on hands and knees or is five months of age, whichever occurs first. Evidence: 1. There was a six month old infant observed in care. The infant's crib was at the highest position for sleeping near the top of the crib and there was a mobile over the crib and another toy attached to the side of the crib. The infant was observed using the crib during the inspection. The infant was observed rolling and turning over. Plan of Correction: The provider removed the mobile and the toy on the side during the inspection and also lowered the crib.
Standard #: 22VAC40-111-350-E Description: REPEAT VIOLATION Based on observation and measurement, the provider failed to ensure that the hot water in the bathroom used by the children was maintained within a range of 105 to 120 degrees. Evidence: 1. The hot water was observed to be steaming hot coming out of the faucet in the bathroom. The temperature was measured with a digital thermometer which read 129 degrees Fahrenheit. Plan of Correction: The provider will have the household member turn the thermostat on the hot water heater down. The provider will use her digital meat thermometer to check the temperature to make sure it is not over 120 degrees.
Standard #: 22VAC40-111-600-B Description: Based on observation, the provider failed to ensure that infants and toddlers spent no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour. Evidence: 1. A six month old infant was observed to be placed in an infant seat (bouncy seat) with a safety strap fastened to secure the child at 9:35 am. The infant was taken out of the infant seat by the provider at 10:18. The infant was placed in a crib to play while awake at 10:20 am. The provider removed the child from the crib at 10:27 after the infant began crying. The provider put the infant into the same infant seat (bouncy seat) at 10:30 until approximately 10:40 am. The infant was taken out at approximately 10:40 am until 11:10 am (30 minutes) when the infant was placed into an infant "Bumbo" seat. The infant was in a confined space more than 30 minutes and did not have at least one hour between confinements. Both the infant seat and the crib are considered confining spaces. The infant was awake during the entire time observed. Plan of Correction: The provider will make sure that an infant is not confined for more than 30 minutes at a time and has more than one hour in between confinements.
