54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today from 9:50am-12:45pm. There were 4 children (11 points) directly supervised by 1 staff at inspector's time of arrival. A second caregiver arrived later on during the inspection. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, outdoors and indoors, singing, and eating lunch (Spare ribs, green beans, sweet potatoes). There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, one of four children records did not have written authorization for emergency medical care in case of an emergency. Evidence: During the inspection, child#3 did not have documentation on file of an authorization for emergency medical care filled out and signed by the parents. Plan of Correction: I will talk to the parents today at pick up time.
Standard #: 22VAC40-111-180-A Description: Based on record review, one of two caregivers did not have documentation of a current report of Tuberculosis Screening form done within two years of the last report. Evidence: During the inspection, Assistant#1 did not have documentation of a current Tuberculosis Screening form. The last TB screening form on file was dated: 10/13/2012 Plan of Correction: I will get a coy of the new TB screening test, which was done in September of 2016
Standard #: 22VAC40-111-690-D Description: Based on observation and staff interview, the diapering surface is not cleaned and sanitized properly after each use. Evidence: Provider states that the diapering surface is sprayed with Lysol after each diaper change. Soap and water and bleach are not being used as required to adequately clean and sanitize the diapering surface. Plan of Correction: Provider will make sure soap, water and bleach is used to sanitize the diapering area.
