An unannounced renewal inspection was conducted today from approximately 12:35pm-1:50pm. There were 5 children (15 points) directly supervised by 2 caregivers. A sample of 3 children?s records, 2 staff records and 2 household member records were reviewed. The children were observed eating lunch, napping, and playing with one another. The outside playing area was well maintained. Zero new staff has been hired since the last inspection. Standard #: 22VAC40-111-60-B
Description: Based on children's files reviewed, children's files were found to be incomplete. Evidence: 3 of 3 children's files reviewed were incomplete, missing information includes: 1) Child A was missing the father's work address 2) Child B was missing an annual update 3) Child C was missing the physicians address and phone number; the address and phone number of 1 emergency contact and the name, address, and phone number for a second emergency contact; proof of identification and a physical within 30 days.

Plan of Correction: I will get the parents to update the files

Standard #: 22VAC40-111-210-A
Description: Based on caregiver's records reviewed, a caregiver failed to obtain 16 clock hours of training annually. Evidence: Caregiver A, completed 12 clock hours of training annually.

Plan of Correction: I will complete the rest of the training.

Standard #: 22VAC40-111-330-A
Description: Based on observation, A landline telephone, excluding a cordless or cell phone was not accessible during the family day home's hours of operation. Evidence: At the time of inspection the landline phone being used was a cordless phone requiring electricity to operate.
