Description: Based on provider interview, the provider failed to have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food which the child is allergic to and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child 6 did not have documentation of a written allergy action plan for a diagnosed food allergy.

Plan of Correction: Provider will request the information from the parent.

Description: Based on provider interview, the provider failed to update the emergency preparedness plan to include reunification for evacuation, shelter in place and procedures for lockdown. Evidence: there were no written procedures to reunite children with a parent or authorized person designated by the parent, no procedures to accommodate infants, toddlers, and children with special needs to ensure their safety during evacuation or relocation, and no procedures for lockdown to isolate children from a security threat.

Plan of Correction: Provider will write procedures to update the emergency preparedness plan.
