An unannounced monitoring inspection was conducted with provider. There were six children present, ranging in age from 1 year old to 12 years old, totalling 13 points. The children were waking from rest, having afternoon snack, and arriving from school. The provider put on Go Noodle music and the children danced. Snack served today: crackers and milk. Provider does not administer medication - Medication Administration Training is required to administer medication to day care children. Two children's files and three household files were reviewed - please see Supplemental Summary for missing items. Provider's first aid and CPR is valid until 1/9/2018. Standard #: 22VAC40-111-910-C
Description: Based on observation, the provider did not prevent children from sharing cups. Evidence: one child drank out of another child's sippy cup during today's inspection.

Plan of Correction: Provider will prevent children from drinking from each other's cups.

Standard #: 22VAC40-111-60-A
Description: Based on review of three children's records, the provider failed to have all required information in each child's file. Evidence: Child A and Child B did not have the home address or home phone number for the custodial guardian. Child 2 did not have the name and policy number of the child's medical insurance. A custodial agreement or placement agreement is required for Child 2 to show guardianship.

Plan of Correction: Provider will request the information from the guardian.
