Technical Assistance:
Discussed with provider background checks, medical records for children, diaper pads.

An unannounced renewal inspection was conducted from 10:09 am to 11:30 am. There was only one child in care (4 points) with one staff person. Child, staff and household member files were reviewed. The provider was observed feeding the child. There is an ample supply of toys and equipment for children. Infant cribs are in compliance with manufacturing dates. Standard #: 22VAC40-111-750-A
Description: Based on observation, a topical ointment was not labeled with the child's name. Evidence: Diaper ointment for Child # 1 was not labeled.

Plan of Correction: I will label the cream.

Standard #: 22VAC40-111-830-C
Description: Based on record review and staff interview, documentation of emergency evacuation and shelter-in-place drills was not maintained. Evidence: 1. The date for the last emergency drill was documented as 6/1/17 on the evacuation drills form. The form was incomplete and there was no record to show the type of drill, time, action taken and method used to alert children. 2. There was no documentation of an emergency drill for the months prior to June 2017. Provider stated that she threw away the papers.

Plan of Correction: Fire and Shelter in Place drills will be conducted and documentation will be saved.

Standard #: 22VAC40-111-50-A
Description: Based on record review and staff interview, the provider did not keep a written record of attendance. Evidence: Child # 1 started care on 10/3/2017. Attendance was not recorded for 10/16/17 through 10/20/17.

Plan of Correction: Attendance will be recorded regularly.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not maintain documentation of a current tuberculosis (TB) screening for each caregiver. Evidence: Documentation of a current TB screening was not found on file for Staff # 1. The date for Staff #1's last TB was 1/30/14.

Plan of Correction: I will find the form and place in file.

Standard #: 22VAC40-111-340-C
Description: Based on observation and staff interview, the bathroom did not contain liquid soap. Evidence: The bathroom next to the diaper changing area did not have liquid soap for hand washing.

Plan of Correction: Liquid soap will be placed in the bathroom.

Standard #: 22VAC40-111-680-A
Description: Based on observation, provider did not wash hands with liquid soap and water. Evidence: Provider did not wash hands before feeding the baby a milk bottle.

Plan of Correction: Hand washing will be done before all meals.
