Comments:
An unannounced monitoring inspection was conducted today from 10:25am-2:00pm. There were 9 children (29 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 6 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site. Children were observed playing freely and eating lunch (Cabbage rice, chicken and green lentils). Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-770
Description: Based on observation, the family day home did not have working batteries for the portable battery-operated weather band radio. Evidence: The provider has a battery-operated weather band radio but did not have working batteries during the inspection.

Standard #: 22VAC40-111-960-C
Description: Based on observation, feeding tables with safety waist straps were not used according to the manufacturer's instructions. Evidence: During the inspection children were observed seating in the feeding table not wearing their safety straps.

Description: Based on record review, 2 children's records did not have documentation of updated immunizations on file. Evidence: Child#1 and child#2, who are both under the age of two, did not have documentation of additional immunizations every 6 months. The most recent dates are: child#1 - 12/19/2016 and child#2 - 08/01/2016

Description: Based on observation, areas and furnishings inside of the family day home, shall be maintained in a clean, safe, and operable condition. Evidence: Two cribs in the home had splintered areas and chipped paint all around the top rails.

Description: Based on observation, a window blind was not kept out of reach of children in care. Evidence: A broken blind with a long cord was observed next to a crib in the napping room accessible to children.

Standard #: 22VAC40-111-320-B
Description: Based on observation, not all unused electrical outlets had child-resistant protective covers. Evidence: During the inspection protective covers were not found in the lower level bathroom, on a wall in the infant area and under a table in the children's eating area. All of these outlets were accessible to children and were located in the programming area of the family day home.

Description: Based on observation and staff interviews, the provider uses a cloth towel that she washes daily instead of paper towels. Evidence: The provider did not have paper towels in the bathroom used by the children. She stated that uses one cloth towel for all children and can not afford paper towels.

Description: Based on observation, children's hands were not washed with soap and water after diapering. Evidence: 3 children's hands were not washed following their diaper changes.

Description: Based on observation, the diapering surface was not cleaned and sanitized after each use. Evidence: The diapering area was not cleaned and sanitized between the three diaper changes while the Inspector was on-site.

Description: Based on record review, the licensee had knowledge that an individual living in the home has an offense in another state outside of Virginia. Evidence: The Sworn Disclosure Statement completed by Household Member # 2 was checked that the individual had been the subject of a founded complaint of child abuse or neglect outside the the Commonwealth of Virginia.

Standard #: 22VAC40-191-60-B
Description: Based on record review, assistant#1 did not have documentation of a Sworn Statement of Affirmation during the inspection. Evidence: Assistant#1 started working at the family day home in August of 2017, but documentation of a Sworn Statement of Affirmation was not found in her file.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, assistant #1 did not have a Central Registry Search within 30 days of employment. Evidence: Staff#1 was hired in August of 2017 and does not have a completed Central Registry within 30 days of employment.

Plan of Correction: We did it on time. The office sent it back to us by mistake. So, we needed it to resend again. They are in the file already. [ sic ]

Standard #: 63.2-1721.1-C
Description: Based on record review, a household member has completed a Sworn Disclosure Statement indicating a disqualifying central registry finding. Evidence: Household member #2 completed a Sworn Disclosure Statement indicating they are the subject of a founded complaint of child abuse or neglect. Sworn Disclosure Statement was completed and signed on 9/5/2017.

Plan of Correction: Nothing found in VA background search, what he wrote on his Sworn was not correct [ sic ]
