54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted on October 26, 2017, between the hours of approximately 1:30 p.m. and 4:00 p.m. Fourteen (14) children were enrolled and ten children were in the care of the provider and one assistant during the on site inspection. Five children's records and two caregivers' records were reviewed and/or updated. No medications were administered. Facility documents and posted information were observed/reviewed. Program observation was completed. Activities observed on this date included: naptime; diapering, restroom and handwashing procedures; Halloween parade; afternoon snack and pick-up/departure of one child. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. The children knew and followed the daily routine and transitions well. Previously cited violations (inspection date - 9/14/2017) were reviewed for compliance; one repeat violation was cited. One exception to compliance was noted on the violation notice. Standard #: 22VAC40-111-100-A Description: Based on review and/or update of five children's records and discussion with the provider on October 26, 2017, documentation of a physical exam report was not on file for one child within 30 days after the first day of attendance. Evidence: 1. Update of child A's record, on this date, determined that a physical exam report was not on file (first date of attendance - 7/25/2017). 2. Caregiver #1 confirmed the lack of physical exam report for child A. Plan of Correction: The child's parent will be contacted and a physical exam report will be obtained.
