An unannounced renewal visit was conducted today between the hours of 10:00 am through 12:30pm with the provider on site. There were 5 children (14 points) with the provider and assistant. A sample of 5 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, singing, hand washing and lunch time. Standard #: 22VAC40-111-780-B Description: Based on record review, the provider did not review with the parent/obtain their signature, for the emergency contact information to ensure the information is correct yearly. Evidence: 1. Child A and Child B enrolled on 09/06/2016 and did not have documentation that a review of the emergency contact information was completed and signed by the parent(s). Plan of Correction: I will have the parent review for any needed changes and sign.
