Technical Assistance:
Technical assistance was provided on the following items during the Inspection: 22VAC40-111-(3)-140 (Substitute providers); 22VAC40-111-(7)-240 (Home maintenance); 22VAC40-111-(12)-920 (Menus); 22VAC40-191 (Background checks)

Comments:
An unannounced renewal inspection was conducted on 2/21/17 from 11:10am - 12:15pm. At the time of entrance there were six children ages 16 months old to four years old (13 points) in care with one caregivers. The provider arrived arrived to the family day home around 11:15 after running an errand. A tour of the home was conducted and children were observed playing inside, completing a learning activity, and eating lunch. Records were reviewed for six children and three caregivers. There is no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of six children's records, it was determined that the provider did not ensure that each child in attendance had a completed physical within one month of attendance. Evidence: 1. The record for child#2 (date of enrollment 8/30/17) did not contain documentation of a physical examination. 2. The provider confirmed that she had not received a physical examination for child #2 , and stated that she has asked the parent of child #2 to provide a physical examination.

Plan of Correction: The provider responded: Child #2 Mother presented record of Physical Record of Child on file as of Friday 11/3/17.

Standard #: 22VAC40-111-30-A
Description: Based on interviews and observation, it was determined that the licensee did not ensure that the facility operated within terms of the current license issued by the department. Evidence: 1. The age range on the current license issued by the department to the facility is two years old through eleven years old. 2. A review of children records indicated that child #5 (date of enrollment 8/30/16) is 16 months old. 3. Caregiver #1 confirmed that child #5 had attended the family day home while she under the age of two years old, even though the license does not allow for children under the age of two years old to be in care.

Plan of Correction: The provider responded: Child #5 Last Day was 10/31/17 Tuesday. Until License is renewed for ages 0 to 11.

Standard #: 22VAC40-111-80-A
Description: Based on a review of six children's records, it was determined that the provider did not ensure that within seven business days of the child's first day of attendance at the family day home, verification of the identity and age is obtained from the parent. Evidence: 1. The record for child #2 did not include documentation to demonstrate the parent had provided verification of the identity and identity of child #2. 2. The provider confirmed that she had not verified the identity and age of child, and stated she has asked the parent of child #2 to provide a birth certificate.

Plan of Correction: The provider responded: Child #2, Birth Record Letter is on File as of Friday 11/3/17.

Standard #: 22VAC40-111-90-A
Description: Based on a review of six children's records, it was determined that the provider did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence: 1. The record for child #2 (date of enrollment 8/30/17) did not contain documentation for immunizations. 2. The provider confirmed that she had not an immunization record for child #2 , and stated she has asked the parent of child #2 to provide an immunization record.

Plan of Correction: The provider responded: Child#2 Mother presented record of Immunization Friday 11/3/17 on file in child?s file

Standard #: 22VAC40-111-150-D-1
Description: Based on a review of three caregiver records, the licensee did not ensure that any caregiver left alone with children had current certification in cardiopulmonary resuscitation (CPR). Evidence: 1. The record for caregiver #2 did not contain documentation to demonstrate caregiver #2 had a current certification for cardiopulmonary resuscitation (CPR). 2. The provider stated that caregiver #2 is scheduled for a training for cardiopulmonary resuscitation (CPR) on 11/6/17.

Plan of Correction: The provider responded: Certification in Cardiopulmonary Resuscitation (CPR) Ms. Angel Chambers Class Date is 11/6/17 Monday for CPR & First Aid. Class receipt on file in Staff Folder. Staff Member Ms Kamyra Green Pending class until Hired Date 12/4/17. Awaiting to Set class for Saturday 12/2/17. As of Nora Schmidt is no longer working with the children. Ms Schmidt is only my Administration Secretary. But is willing to take the CPR Classes on 12/2/17.

Standard #: 22VAC40-111-150-D-2
Description: Based on a review of three caregiver records, it was determined that the licensee did not ensure that any caregiver left alone with children had current certification in first aid. Evidence: 1. The record for caregiver #2 did not contain documentation to demonstrate caregiver #2 had a current certification for first aid. 2. The provider stated that caregiver #2 is scheduled for a training for first aid on 11/6/17.

Plan of Correction: The provider responded: Certification in Cardiopulmonary Resuscitation (CPR) Ms. Angel Chambers Class Date is 11/6/17 Monday for CPR & First Aid. Standard #: 22VAC40-111-170-A
Description: Based on a review of three caregiver records, it was determined that the licensee did not ensure that the provider shall obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form. Evidence: 1. The record for caregiver #2 did not contain documentation of a Tuberculosis Screening. 2. The provider and caregiver #2 both confirmed that caregiver #2 had not completed a Tuberculosis Screening.

Plan of Correction: The provider responded: Ms. Angel Chambers TB test reading review is Monday 11/6/17. Will have paper work on file Tuesday 11/7/17.

Standard #: 22VAC40-111-510-A
Description: Based on observation and interviews, it was determined that each is provided with an individual crib, cot, rest mat, or bed for resting or napping. Evidence: 1. Upon entry to the family day home, the Licensing Inspectors observed child #5 sleeping in a pack-n-play. 2. The provider confirmed that child #5 was not sleeping on a crib, cot, rest mat, or bed as required by the standards, but that she does have cots and cribs available for children to use during resting or napping.

Plan of Correction: The provider responded: We have cots and individual cribs available. the Pack-N-Play is no longer in the Day Care Facility, as of 10/31/17.
