Today an unannounced Monitoring inspection was conducted with Daphne Moore. This visit took place in the morning/afternoon between 10:45am and 12:40pm. There were 5 children present with the provider for a total of 14 points within the supervision guidelines. The children were observed having a snack, doing their morning lesson, watching TV, washing hands and beginning lunch. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Description: Based on observation, emergency evacuation drills were not practiced monthly as required. Evidence: There is no documentation for emergency evacuation drills practiced for the months of June, July, August, September and October.

Plan of Correction: I will find our more current form. The form was taken out. We do practice them but the form is misplaced.

Description: Based on observation and interview, the weekly menu was not posted. Evidence: At the start of the inspection, the food menu for this week was not posted. The Provider confirmed that she had not yet posted the menu.

Description: Based on record review, children's records did not contain all required information. Evidence: Child #3 file is missing the medical insurance policy number.

Plan of Correction: I will ask the parents to add this information to the form.

Description: Based on observation, potentially poisonous materials were left accessible to children. Evidence: There was a bottle of air freshener left on the shelf next to the changing table, which was accessible to the children.

Plan of Correction: This was corrected during the inspection. The air freshener was moved to a locked closet.

Description: Based on observation, small appliances were not unplugged when not in use. Evidence: At the start of the inspection, the providers coffee maker and toaster oven were left plugged in.

Description: Based on observation, protective covers were not installed on all unused electrical outlets. Evidence: There was one protective cover missing from the outlet near the front window. There was one protective cover missing from the outlet near the front door.
