Standard #: 22VAC40-111-960-E Description: Based on observation, 3 infants were present however infant bottles did not include the date. Evidence: 3 infant bottles did not include the date. Plan of Correction: Provider stated that she will make sure all bottles have the date.
Standard #: 22VAC40-111-240-A Description: Based on observation of the family day home, the following areas in the home were not being maintained in a clean, safe and operable condition. Evidence: Chipped/peeling paint were found near the steps and in the main childcare area along the walls in the basement. Plan of Correction: Provider will have them repainted.
Standard #: 22VAC40-111-250-C Description: Based on observation and interview with Provider, items were found tied across the top of a crib occupied by a child over the age of five months. Evidence: Upon arrival, a piece of cloth material with a toy attached was found hanging across the top of the crib with a toy dangling from it. An infant was observed in the crib who was able to push up and 7 months of age. Plan of Correction: Provider immediately removed the items from the crib and stated that she will make sure that nothing is attached to the crib.
Standard #: 22VAC40-111-540-A Description: Based on observation, high chairs occupied by children did not have the safety strap securely fastened and were not being used according to the manufacturer's instructions. Evidence: Upon arrival, 4 children were observed sitting in high chairs however the safety strap were no being used and securely fastened. Plan of Correction: Provider immediately put the safety strap on all of the children.
