An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 3 children, totaling 6 points. One additional child attends on a part time schedule. Children's records, new caregiver records, the indoor and outdoor physical plant, emergency preparedness items, administrative documents, and children's nutrition were inspected today. The children were observed playing with toys, interacting with each caregiver, and eating lunch from home. Pleasant and friendly interactions were observed between all caregivers and all children in care. This inspection was completed between approximately 11:40 am and 1:00 pm. Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: The record for assistant caregiver A does not include emergency contact information.

Plan of Correction: I will make sure she fills out the form today.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that four records are not complete. Evidence: 1. Child record #1 and #2 do not include: a second emergency contact; a signed "Information for Parents" document; the provider's signature indicating that she has viewed the child's original proof of identification; the parent's signature authorizing the provider to obtain emergency care if needed; a signed "Provisions of the Emergency Plan" document; a signed "Declaration of Liability Insurance" document. 2. Child record #3 and #4 do not include: a signed "Provisions of the Emergency Plan" document; a signed "Declaration of Liability Insurance" document; the provider's signature indicating that she has viewed the child's original proof of identification.
