Comments:
The following inspection took place between the hours of 10:30 am through 12:15pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 10 children (29 points) with 2 caregivers. A total of 10 children's files and 4 caregiver/household member's files were reviewed. The children were engaged in the following: toy play, potty training, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-960-E
Description: Based on observation, infant bottles were not properly labeled. Evidence: 1.) 4 bottles were missing children's names. 2.) 8 bottles were missing the date.

Standard #: 22VAC40-111-60-B
Description: Based on a review of records, children's records did not contain the required information. 1.) 4 out of 10 records were missing parent work addresses and/or work phone number. 2.) 2 out of 10 records were missing 1 of 2 required emergency contacts. 3.) 4 out of 10 records were missing medication insurance and policy numbers.

Description: Based on a review of records, the written of information form did not contain the required information. Evidence: The written information to parents form in the records of 8 out of 10 children were missing: 1.) the amount of time per week that an assistant is scheduled to care for the children, and 2.) a daily schedule.

Description: Based on observation, areas of the home were not maintained in safe condition. Evidence: 1.) Dozens of unstable bricks were observed accessible to children underneath the deck beside the outdoor play area. 2.) Rust, and hard cracked plastic were observed on the chains of 3 swings.

Description: Based on observation, poisonous substances were not stored inaccessible to children. Evidence: A cleanser was observed in an unlocked cabinet without a safety latch. A cleanser was also observed in an unlocked storage closet outdoors near the play area.
