54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today from 11:20am-1:00pm. There were 8 children (15 points) directly supervised by the provider. The home, 8 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, packing a spider Oreo cookie and pretzels snack they had prepared during their Halloween celebration, and parent pick up. The programming area was well supplied with books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-A Description: Based on record review and staff interview, evacuation drills were not practiced monthly. Evidence: Evacuation drills were not practiced in the month of June and in the month of September, only shelter-in-place drills were practiced during these months. Plan of Correction: I will do evacuation drills monthly.
Standard #: 22VAC40-111-50-A Description: Based on record review, there was no documentation of a written record of children's daily attendance. Evidence: A record of daily attendance was missing for the week of 10/24-10/27/2017. Plan of Correction: I recorded missing attendance for the week of 10/24/17 - 10/27/17. I'll keep attendance now and later.
Standard #: 22VAC40-111-330-A Description: Based on observation and discussion, a landline telephone was not available, operable and accessible. Evidence: The provider confirmed that the landline telephone could not be located during the inspection. Plan of Correction: We connected a landline telephone.
Standard #: 22VAC40-191-40-D-5-a Description: Based on record review, a household member did not have documentation of a Central Registry check within 30 day of the person becoming 14 years old. Evidence: Household member#1 (DOB:4/30/02) did not have documentation of a Central Registry check within 30 days of the person becoming 14 years old. Plan of Correction: We submitted the Central Registry documentation to the Department of Social Services.
