Technical Assistance:
Discussed training resources, background checks, diapering areas.

An unannounced monitoring inspection was conducted from 10:00 am through 12:45pm. At the time of entrance there were a total of 12 children (35 points) with 3 caregivers. A total of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, circle time, lunch and singing activities. Children had chicken, rice and steamed broccoli for lunch. There is an ample supply of toys and supplies for the children. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-800-B
Description: Based on record review, provider did not update the emergency plan annually. Evidence: The emergency plan was last updated on 5/6/16.

Plan of Correction: I will update the emergency plan.

Standard #: 22VAC40-111-60-B
Description: Based on record review, a child's record did not contain names of persons authorized to pick up the child. Evidence: Child # 1 and Child # 2's records did not have authorized persons to pick up listed on the record form.

Plan of Correction: I will get the information from the parents.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider did not obtain additional immunization records for children. Evidence: 1. The dates of the last immunization records for Child # 1 obtained from parents were from 2015. There were no additional immunization records on file.

Plan of Correction: I will ask the parents to bring in the updated shot records for the child.

Standard #: 22VAC40-111-210-A
Description: Based on record review and staff interview, caregivers did not obtain 16 clock hours of annual training. Evidence: 1. Three caregiver files were reviewed. There was no documentation of training found on file for all three caregivers. 2. Staff #1 stated that 3 of 3 caregivers had not done any training for the year. The last training completed was in March 2016.

Plan of Correction: We will get the trainings done.

Standard #: 22VAC40-111-690-D
Description: Based on observation and staff interviews, the diapering surface was not cleaned and sanitized after each use. Evidence: 1. A bottle of disinfecting wipes were found next to the diaper changing area. Staff # 1 stated that they use the wipes to clean the surface after each diaper change. 2. There was system in place for cleaning and disinfecting the diaper changing area after each diaper change. Staff # 1 stated that she did not know that the area needed cleaning with a soap and water solution.

Plan of Correction: I will correct the cleaning and sanitizing procedures.
