An unannounced renewal study was conducted today. There are 8 children present ( 26 points) with 3 caregivers. 3 household, 1 assistant and 6 children's files were reviewed. See violation notice for missing items. Children were observed playing with a variety of toys during today's inspection. The outside play area was not inspected due to inclement conditions. Emergency escape drills, first aid supplies, fridge temperature and bathroom supplies are all in compliance. Description: Based upon a record review, children's records are not up to date. Evidence: 2 of 6 files reviewed were missing the annual update to the record. (Children # 8, 7)

Description: Based upon a review of records, children's records do not contain all required information. Evidence: 1 of 6 records reviewed was missing the health insurance policy number. (Child #1)

Description: Based upon a review of records, one child's record does not include an immunization record, prior to the first day of attendance. Evidence: Child # 4's record has no documentation of the immunization record. (First date of attendance: 11/13/17)
