Comments:
An unannounced monitoring inspection was conducted on 11/21/17 from 11:50am - 12:55pm. At the time of the inspection there were 12 children in care, ages 1year old to five years old (29 points), with three caregivers present. Records were reviewed for five children and three caregivers/household members. Children were observed during rest period. The provider stated there are no medications being administered at this time. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation and interview, it was determined that the provider did not ensure there is a a working portable battery-operated weather band radio available to caregivers at all times. Evidence: 1. The weather band radio available for viewing during the inspection was not operable by batteries during the inspection. It was operational by use of a hand crank when batteries are not available. 2. The provider confirmed that there was not a working battery-operated weather band radio during the inspection.

Plan of Correction: The provider responded: I will ensure that there is a working battery operated weather band radio at all the times there are children in care.

Standard #: 22VAC40-111-90-C
Description: Based on a review of five children's records, it was determined that the provider did not ensure that documentation of additional immunizations is obtained every six months for children under the age of two years old. Evidence: 1. The record for child #4 (date of birth 6/28/16) contained an immunization record dated 1/5/17. 2. Caregiver #1 reviewed the record for child #4, and confirmed that an updated immunization record had not been received for child #4.

Plan of Correction: The provider responded: The parents of child #4 will be asked to provide an updated immunization record. Immunizations records will be updated every six months for all children 2 years old and younger.

Standard #: 22VAC40-111-350-E
Description: Based on observation, it was determined that the provider did not ensure that the hot water at taps available to children are maintained with in a range of 105 to 120 degrees Fahrenheit. Evidence: 1. The hot water tap in the bathroom located in the the family day home that is used by the children was 124.7 degrees Fahrenheit. 2. The provider viewed the thermometer and confirmed that the hot water temperature was not within the required temperature range.

Plan of Correction: The provider responded: I will turn down the temperature on the water heater and regualrly check the temperature of the water in the bathroom.
