12 children were in care during the inspection total of 26 points. The age range of the children was 15 months to four years old. Children were observed during free play, lunch time and rest. No children are on medications. Two staff and five children's records were reviewed. Inspection time 12:45am to 2:30pm.
Standard #: 22VAC40-111-990-B Description: Based on observation and interviews with the provider and one assistant not all transportation requirements were followed. Evidence: There was no documentation of children's emergency contact in the van used to transport children, The was no emergency supplies in the van. Plan of Correction: I will make sure all emergency supplies are in the van.
Standard #: 22VAC40-111-570-A Description: Based on observation and interviews the assistant was caring for more than 16 points. Evidence: The assistant was caring for 26 points alone for approximately 15 minutes. Plan of Correction: I will never leave the assistant alone again.
Standard #: 22VAC40-111-690-D Description: Based on observation the diapering table was not cleaned. Evidence: The staff did not use soap and water to clean to the diaper pad before sanitizing. Plan of Correction: We will use soap and water before sanitizing it.
