54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced renewal visit was conducted today between the hours of 9:00am through 11:20am with the provider on site. There were 4 children (9 points) with the provider and her assistant. Five children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, listening to music, and snack time. The provider and assistant are both certified in CPR and First Aid. The provider is also MAT trained to give medication. The home is clean and organized with an abundant supply of books and toys. Evacuation drills and emergency supplies were inspected. There were no medication on site today. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-100-A
Description: Based on review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child C started at the family day home on 10/02/2017. At the time of the inspection, the provider did not have a copy of child C's physical examination and it has been more than 30 days after the first day of attendance.

Plan of Correction: I will ask the parent to get a copy of the physical for his record.

Standard #: 22VAC40-111-60-B
Description: Based on review, one of five children's records were not complete. Evidence: 1. Child A was missing one parent's address, place of work, work address and work phone number, and one emergency contact name, address and phone number.

Plan of Correction: I will ask the parent for the information today.

Standard #: 22VAC40-111-70-A
Description: Based on review, one of five children's records did not contain a parent's signature on the information for parents form by the first day of attendance. Evidence: 1. Child C began enrollment on 10/02/2017. The child's file did not contain a signed information for parents form as required before the child's first day of attendance.

Plan of Correction: I will give the parent the form tommorrow and ask for the signature.

Standard #: 22VAC40-191-60-C-2
Description: Based on review, the provider does not have the Assistant's central registry findings within 30 days of employment. Evidence: 1. The Assistant began employment at the family day home 08/31/2017. The Assistant's file does not contain the central registry findings and she has been employed more than 30 days.

Plan of Correction: I had sent it and it was returned due to a missing signature and I resent it a few weeks ago. If I do not receive it in the next week, I will resend a new one.
