An unannounced renewal inspection was conducted on November 21st 2017 between 11:00am and 2:00pm. There were 9 children in the direct care of 3 staff members. Children were observed eating lunch, napping, working on art projects, listening to stories, and playing with toys. The day care is spacious, clean, and organized. Standard #: 22VAC40-111-120-B Description: Based on record review, 1 of 3 staff records reviewed does not have all of the required information. Evidence: Staff #3 record does not have documentation that two references were contacted; including dates of contact, name of persons contacted, firms contacted, results, and signature of the person making the call. Plan of Correction: I will redo them to place them in the file.
Standard #: 22VAC40-111-40-A Description: Based on direct observation and record review, the family day home did not ensure that the total number of children recieving care at one time does not exceed the maximum licensed capacity of the home. Evidence: During the inspection there were 9 children in care. The capacity for the family day home is 7 children. The provider submitted zoning documentation to the licensing office on November 15th 2017 that would allow for a maximum of 12 children. in care, but the license has yet to be adjusted. Attendance records from October 30 to November 15 2017 were reviewed and the family day home was over capacity 8 of 12 days. Plan of Correction: Paperwork was approved from the Fairfax County Planning Commission on October 23 allowing me to have 12 children in care at any one time. I understood that the zoning office would send the document to the licensing office.
Standard #: 22VAC40-111-180-A Description: Based on record review, 1 of 3 staff records reviewed did not have documentation that each caregiver had a current report of Tuberculosis screening every two years from the date of the first screening. Evidence: Staff #2 record last documented Tuberculosis screening was conducted May 8th, 2015 Plan of Correction: Staff will turn in paperwork tomorrow morning and I will place it in the file.
Standard #: 22VAC40-191-40-D-1-b Description: Based on record review, 1 of 3 staff records did not contain complete background checks for an employee who works with children. Evidence: The central registry background check for Staff #2 does not contain the second page of the document, which shows the date in which the background check was conducted and by whom. The document was date stamped as received in the Office of Background investigations n December 30th 2014, February 25th 2015, and March 18th 2015. Plan of Correction: Will resubmit the background check.
Standard #: 63.2-1720.1-B-3 Description: Based on a review of 3 staffing record, 1 of 3 records did not contain a request or the results of a central registry search for employees that have lived outside of the state of Virginia within the last five (5) years. Evidence: Staff #3 disclosed on the sworn statement that they have lived outside the state of Virginia within the last five years. Plan of Correction: I will reach out to Texas and get the necessary paperwork.
