An unannounced monitoring inspection was completed from 2:00pm until 3:00pm. At the time of the inspection there were 8 children in care for a total of 20 points with the provider and one assistant present. A sample of 5 children's records and 5 provider/assistant/household member records were reviewed. The children were observed napping during the inspection and one infant was being held and bottle fed by the assistant. The tour consisted of the indoor child care area, the downstairs area of the home and the outdoor play area. First aid and emergency supplies were reviewed, as well as the emergency preparedness plan and other documents pertaining to the family day home. The provider does not administer any medications at this time but is MAT certified. Firearms were observed to be in a locked gun cabinet in a room that is not used for the care of children. Ammunition is stored in a separate locked cabinet in the same room. One violation in the area of administration is identified on the violation notice and was discussed with the provider during the exit interview.

Description: Based on record review and interview, the provider did not obtain documentation of a physical exam by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. The record for child #1 does not contain documentation of a physical examination. a. Child #1 has an enrollment date of 5/30/17 and was in care on the day of the inspection. 2. The provider confirmed that the record for child #1 does not contain documentation of a physical exam.

Plan of Correction: The provider stated this was an oversight and will notify the parent today at pickup that documentation of a physical exam is needed.
