1/29/2017 10:00am Departed 12:20pm An unannounced monitoring inspection was conducted today. Seven children were in care for a total of 16 points. Children's and staff records were reviewed. Standard #: 22VAC40-111-730-D Description: Based on observation, the facility failed to securely lock all medication. 1. Family medication, including ibuprofen and vitamins, is stored on top of the refrigerator. Plan of Correction: The provider will lock all family medications.
Standard #: 22VAC40-191-40-D-1-b Description: Based on the review of three adults' files, the facility failed to conduct all required background checks. Evidence: 1. There is no CPS Clearance on file for staff #3. Plan of Correction: The provider mailed the CPS Clearance on 11/13/2017 and will forward copies to the inspector.
Standard #: 22VAC40-191-40-D-1-c Description: Based on the review of three adult files, the facility failed to repeat criminal background checks before three years since the date of the original form. Evidence: 1. There is no repeat criminal background check on file for staff #1 and 2. Plan of Correction: The provider mailed the CRCs on 11/20/2017 and will forward copies to the inspector.
