Discussed the recent communication regarding fingerprint based background checks for child day programs.
The licensing inspector conducted an unannounced monitoring inspection on Thursday, November 30, 2017 from 08:30am to 10:30am. The children were observed playing with toys and finishing their breakfast inspector?s arrival. Later they were observed having alphebet time and playing outside. Today, there were five children, totaling 13 points, present with two caregivers. Breakfast snack consisted of oatmeal and applesauce. Five children?s records and two caregivers? records were reviewed during the inspection. Medication is not being administered. Transportation is provided. Last emergency drill:10/27/2017 Last shelter-in-place drill: 10/27/2017 Emergency Plan Reviewed: 05/01/2017 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records on 11/30/2017, the provider did not ensure that five out of five records contained the required information. EVIDENCE: 1. The records of Child #1 and Child #2 are missing addresses for the two emergency contacts listed. 2. The record of Child #5 is missing the telephone number for the place of employment listed for the child's father. 3. The records of Child #1, Child #2, Child #3 and Child #5 are missing the medical insurance policy number. 4. The records of Child #3 and Child #4 are missing documentation for a second emergency contact. 5. The record of Child #5 is missing an address for the second emergency contact listed. Plan of Correction: When the parents come in today to pick the children up, I will have them complete the missing information.
