Today's visit was a renewal inspection. The inspection began at 11:15n and concluded at 3:00pm. There were four children present with two providers and the points total for the visit was ten. During the inspection the outside area was viewed, lunch time was observed, nap time was observed, five out of twelve children's records were reviewed, the licensee's records was reviewed, two assistant's records were reviewed, emergency supplies were viewed, a vehicle used for daily transportation was viewed, the emergency plan was viewed, and some additional policies and procedures were reviewed. Your renewal application has been received and once it has been deemed complete and today's inspection is completed a recommendation regarding your license can be made. Standard #: 22VAC40-111-60-B Description: Based on the inspector's review of five out of twelve children's records, the provider failed to ensure a portion of the records had all of the required information. Evidence #1: The record for child #4 indicates health insurance and there is no policy number in the record. Evidence #2: The record for child #5 is the physical address for one out of two required emergency contacts. Plan of Correction: The information will be obtained and added to the records.
