This unannounced monitoring inspection was conducted from 11:10 am until 2:00 pm. At the time of the inspection four children and the provider were present. The sample size consisted of five children's records and the provider's record. Children and the provider were observed during free indoor play, during an art activity, singing, story time, clean-up, hand washing, lunch preparation, and nap. The violations from the previous inspection were reviewed for current compliance. No repeat violations were found. Violations from today's inspection can be found on the violation notice. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider failed to obtain documentation of a physical examination for two out of five children prior to the first date of attendance or within 30 days after the first day of attendance. Evidence: 1. The records for child 1 and child 2 (siblings) did not contain documentation of a physical examination. The start date for the two children was 8/16/17. 2. The provider verified she did not have documentation of the two children's physical examination. Plan of Correction: Documentation of physical examinations will be obtained from each child's parents/guardians.
