Comments:
This unannounced monitoring inspection was conducted from 10:00 am until 12:00 pm. At the time of the entrance ten children were in care with the provider and an assistant for a total of 28 points. The sample size consisted of five children's records and the review of the provider's and assistant's records. Children and staff were observed during TV time, free play, educational lesson, arrival, dancing, lunch, diapering, hand washing, transitions, and behavioral management. Standard #: 22VAC40-111-60-B
Description: Based on a review of five children's records, the provider failed to obtain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached for one child's record.

Evidence:

The record for child 3 only contained the name, address and telephone number of one designated person to contact in case of an emergency if the parent cannot be reached. The first date of attendance for child 3 was 6/26/17.

Plan of Correction: The parents on child 3 will be advised to provide another emergency contact to be added to the record.
