Technical Assistance:
Discussed with the provider getting additional information from parent about need for epi-pen. Also discussed importance of bringing violations into compliance before license renewal.

An unannounced monitoring inspection was conducted from 2:00 pm to 4:00 pm with the provider and her assistant. There were seven children ranging in age from eight months to four years old for a total of 18 points. Children were observed napping, having snack of applesauce, goldfish and water, and playing with Montessori manipulative materials. Seven children's records were reviewed - see violation notice. Standard #: 22VAC40-111-760-A-1
Description: Based on interview with the provider it was determined that the first aid kit is not complete. Evidence: there was no triangular bandages in the first aid kit and there was no First Aid book/manual.

Plan of Correction: The first aid kit will be complete.

Standard #: 22VAC40-111-790
Description: Based on observation and interview with the provider, it was determined that there is no phone number listed for the emergency backup person.

Plan of Correction: A phone number for an emergency backup person will be posted.

Standard #: 22VAC40-111-830-A
Description: Based on an interview with the provider it was determined that the emergency evacuation drills and the shelter in place drills have not been conducted. Evidence: There was no documentation of an emergency evacuation drill or shelter in place drill.

Plan of Correction: Emergency escape drills will be done monthly and shelter in place drills will be done twice a year.

Standard #: 22VAC40-111-60-B
Description: Based on a review of the files for 7 enrolled children it was determined that all of the files were incomplete. Evidence: The following information was missing from the children's files: Information for Parents, Liability Insurance Information sheet, Emergency Plan for parents, medication decision and also child E and F did not have a phone number for one emergency contact, child G did not have a medical insurance policy number, and child D did not have a second emergency contact person.

Plan of Correction: All children's files will be complete no later than 1/15/18.
