Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, the new definition of a serious injury, and a review of changes to background checks with regards to expiration dates, new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years.

The following inspection took place between the hours of 10:30 am through 1 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced renewal inspection there were a total of 9 children (26 points) with 2 caregivers. A total of 9 children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: toy play, centers, movie time, music, song, games, diapering, bathroom breaks, hand washing, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-780-B
Description: Based on review, 5 of 9 children's files reviewed indicated that the annual review with the parents of their emergency contact information (required in 22 VAC 40-111-60 B 2 to ensure the information was correct), and to obtain the parent's signed acknowledgment was not conducted. Evidence - 1. On the date of inspection (12/07/2017), the following children had documented reviews older than one year: Child #1, Child #2, Child #3, Child #4, and Child #5. 2. The dates of the most recent acknowledgement were: Child #1 (02/03/2016), Child #2 (10/25/2016), Child #3 (08/31/2015), Child #4 (11/19/2016), and Child #5 (11/19/2016).

Plan of Correction: All applicable emergency contact information shall be reviewed, (if needed) updated, and the form shall be signed in order to acknowledge this review by the parent.

Standard #: 22VAC40-111-100-A
Description: Based on review and interview,1 of 9 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (12/07/2017), documentation of a physical examination was not available for review for Child #8. 2. The Provider stated (during an interview conducted on the date of inspection) that the start date for Child #8 was 10/16/2017. 3. Child #8 was in attendance on the date of inspection.

Plan of Correction: The Parent of Child #8 shall be contacted and the missing documentation shall be obtained.
