Comments:
An unannounced monitoring inspection was conducted from 10:45 am - 12:15 pm. There were ten children in care with the provider and two assistant's ages 3 months, 6 months, 6 months, 12 months, 12 months, 18 months, 3 years, 3 years, 3 years, and 4 years for a total of 31 points. The children were observed participating in structured center play and eating lunch. Lunch is provided from home. Ten children's records were reviewed. The provider and two assistant record's were reviewed. The provider is current in CPR and First Aid Training. It is reported by the provider that there are no medications being administered to children in the family day home at this time. Medication Administration Training (MAT) is required to administer medications to children in the family day home. The most recent emergency escape drill was conducted 12/5/17. Standard #: 22VAC40-111-100-A
Description: Based on observation of ten children's records it was determined that not all records contained documentation of a physical within the first 30 days of enrollment. Evidence: Child B with a start date of 02/13/2017 did not have documentation of a physical on file.

Plan of Correction: The provider will request a copy of an up to date physical from the parents of Child B.

Standard #: 22VAC40-111-90-A
Description: Based on review of ten children's records it was determined that not all records contained documentation of immunizations before the first date of enrollment. Evidence: Child A with a start date of 7/12/17 did not have documentation of immunizations on file.

Plan of Correction: The provider will request an up to date immunization record from the parents of child A.

Standard #: 22VAC40-111-170-A
Description: Based on review of two assistant record's it was determined that not all records contained documentation of a TB (Tuberculosis) screening prior to coming into contact with the children. Evidence: Staff A with a start date of 10/18/17 did not have documentation of a TB screening on file.

Plan of Correction: The provider will request a copy of the TB screening for Staff A.
